What you’ll do

• Care for low-acuity cardiac patients via telehealth

• Adjust medications, track progress, and follow clear care pathways

• Partner daily with cardiologists you trained with onsite

• Educate patients on lifestyle changes and follow-up care

• Use EMR and remote tools to keep care smooth and coordinated

Must-haves

• Nurse Practitioner License — Active PA license highly preferred; if not, must be willing to obtain

• 3+ years outpatient or general cardiology experience (required)

• Valid DEA registration

• Medicare/Medicaid enrollment

• Tech comfort and strong communication skills
