Role Snapshot

What you’ll do

• Conduct face-to-face hospice evaluations

• Support attending physicians and IDG (interdisciplinary group)

• Manage pain and symptom control plans

• Document care progression and patient eligibility

• Visit patients in homes or local facilities

• Collaborate with hospice team to meet patient goals

• Counsel families through end-of-life decisions

Must-haves

• Completed accredited Nurse Practitioner Program

• Active Illinois NP and RN licenses

• Illinois Controlled Substance License + DEA registration

• 2+ years in hospice, palliative, oncology, or symptom management

Nice to have

• Familiarity with EMR charting and IDG workflows
