What you’ll do

• Own your patient panel (no rushing)

• Manage chronic conditions + do full assessments

• Close care gaps (HEDIS and STAR measures)

• Lead care planning with your team

• Keep families informed and supported

Must-haves

• Active, unrestricted APN license in Ohio

• Board certified + Medicare credentialed

• 6+ months skilled nursing or long-term care experience

• Comfortable with digital tools + team-based care

• Willing and able to travel regionally as required
